Program Evaluation (Degree Progress Reports)

Below you will find downloadable blank versions of the program evaluation forms for Northern's degree programs.  Forms such as these, updated with each student's completed courses and requirements, are sent to each current master's degree seeking student prior to the start of each academic term.  They have proven useful in academic accounting of credit and requirements already completed, and as a guide in determining which requirements and how many credits still need to be completed in order to successfully complete a given degree. Your advisor will have computer access to your Degree Progress Report when you request academic advising. All final course designation are determined by the Registrar according to course classification.
